Required Forms
After your registration and payment are received, an e-mail confirmation will be sent. A waiver agreement and medical
authorization form is required. In the event you don't have an e-mail address, please request that the required forms be mailed to you.
- NO CHILD WILL BE ALLOWED TO PARTICIPATE UNTIL ALL FORMS ARE COMPLETED.
- Please bring completed forms to the first day of camp. Camp check-in will open at 7:45am each Monday to expedite this process.
- Please DO NOT mail these forms back to our office.
- PLEASE NOTE: A physician’s authorization is required.
